Upon booking your appointment for our DPN services, you will receive an email and text message with instructions for your visit, including links to the necessary intake forms:
Intake Form (required; first visit only)
Consent & Liability Waiver (each visit; effective July 2024)
Photo/Video Consent for the use of images in marketing and content materials (optional; one-time)
Please note our appointment policies state that the required form(s) must be completed at least 2 hours before your visit. A delay in completing the form(s) could result in a loss of deposit if the appointment needs to be rescheduled.
SLS requires before and after photos/videos for general record-keeping purposes. Rest assured, photos/videos will not be used for marketing or content creation without a signed photo/video consent form.

To secure your appointment, a non-refundable deposit is required in accordance with our appointment policies. The balance payment is due on the day of service.

Please note that a 3% fee will be applied for credit/debit card payments. You can avoid this fee by choosing to pay via Apple Pay, Cash App, or Venmo.
10 min late - $15
20 min late (with prior notice) - $25
20+ min late (without prior notice) - must reschedule; new deposit required
1st No Show - 100% value of the deposit (50% of the full-service charge)
2nd No Show - 100% value of the deposit (50% of the full service charge). Must pay the full service charge when rebooking.
Reschedule 24+ hours in advance - free (no new deposit required)
Reschedule less than 24 hours - new deposit required
Reschedule (Sunday Appointment) 48 or less hours in advance - new deposit required
More than one timely Reschedule (for the same appointment) - $30 Rescheduling fee
These appointment policies are in place to ensure smooth scheduling for services and to provide the personal dedicated 1:1 space for all clients.
Payment, Late, No Show & Rescheduling policies apply to services booked in relation to Groupon vouchers. The applicable appointment policies will be applied to the value of the voucher. Voucher redemption codes must be provided at the time of service booking. Otherwise, a deposit of 50% of the regular service price is due at the time of booking. Appointments will not be confirmed without a valid redemption code or deposit. Must call or text to book a Groupon service.
To ensure a smooth experience with our appointment policies, please wear comfortable clothing and avoid make-up, especially for any DPN services or facial treatments. It's best to schedule your service at least 48-96 hours before a special event.
Depending on the type of service requested, such as skin tag removal and chemical peels, please refrain from having other treatments in the same area 2 to 7 days prior for your health and safety.
If you have any questions, consult with SLS. Additionally, ensure SLS has up-to-date information regarding any allergies, health considerations, and your goals before your service.
Wear comfortable clothing and prepare for your appointment, keeping in mind our appointment policies. Gently exfoliate at least 24 hours prior to your wax session.
For first-time waxing in an area, hair should be the length of a grain of rice. If you are sensitive, you may take Advil or Tylenol 30 minutes prior (optional)*. We recommend booking your service at least 48 hours before a special event.
*SLS recommends consulting with your doctor before taking any medication.
